Primary Health Care


Primary Health Care allows patients to access a doctor or nurse and includes things such as routine physical exams, preventative care, screenings, immunizations, and pregnancy care. We now have limited availability for Physical Therapy as well as Podiatry here in Fort Hall.

We also offer Department of Transportation physical exams for Commercial Driver's Licenses.

We have a limited, small pharmacy for basic needs and we do not dispense narcotics at this location.

The Shoshone-Bannock Community Health Center is staffed with attending providers, nurses and specialists from Southeastern Idaho.

Basic medical services do not require an appointment, however certain services require an appointment, feel free to call us today to ask.

We are your new alternative in Fort Hall Community Urgent Care. Walk ins welcome, no appointments required during normal operating hours. We are open late, open weekends, and open most holidays.

Purchased and Referred Care

If a patient has a medical need beyond our scope, we can write a Purchased and Referred Care referral, pending eligibility


  • For medical needs outside the scope of services of the Community Health Center, our providers can refer care to outside providers.

  • We can do referrals for specialty providers as well, for instance to a dermatologist following PRC processes and rules.

  • Eligible patients can receive their medications at the PRC contracted pharmacies for free. In Pocatello we use Snell's Pharmacy and in Blackfoot we use the Pharmacy Shop.

Our Location

Located in the old IHS clinic – building 70 – on Navajo Road. It is one block due West of the IHS Clinic, drive around the Justice Center and make a left on Navajo Drive, its a long yellow building with a red roof.
